Open the call with "Vorrei prendere un appuntamento con il dottor Rossi." Vorrei keeps it polite, and prendere un appuntamento is the fixed phrase for a new booking.

Mi fa male + body part
For pain in one spot the body part does the hurting: "Mi fa male la gola da lunedì." If two things hurt it becomes mi fanno male, as in mi fanno male le gambe.

“My throat has hurt since Monday.”
When to use it: The first thing you tell the doctor
What you’ll hear back: Deve riposare qualche giorno e bere molta acqua. (You need to rest for a few days and drink plenty of water.)
